To make an appointment, please call our office during normal business hours at 615.790.0567, and you will be directed to a patient services representative who will assist you.
We want to provide all of our patients with great customer service while making sure that each receives the medical attention they need to be well. The following tips will help ensure that you make the most of your appointment:
Plan on arriving 15 to 20 minutes before your appointment to fill out any necessary patient forms.
As a courtesy, we allow a 15-minute grace period for appointments at the Health Center (Suite 319). For the Counseling Center (Suite 202) or Psychiatry (Suite 319), a 10-minute grace period is allowed. If you are later than the allotted grace period for your appointment, you may be asked to reschedule or have to wait for the next available appointment that day.
For your convenience, walk-in appointments are available in the Health Center (Suite 319) for adult and pediatric acute care medical needs. Please call ahead for hours and wait time.
What to Bring
Please bring the following:
One form of identification (driver’s license, passport, state issued ID)
Current health insurance card, if applicable
Current pharmacy card, if applicable
Proof of income if you are uninsured
A list of current medications including the dosage or bring the actual prescription bottles
Previous medical records, if you are a new patient
