For your first appointment, please bring a list of all current medications as well as the pill bottle prescribed, so the drug name and dosage can be added to your chart. At each visit we will review your medications for changes. If there are changes, please bring the changes and the pill bottle prescribed.

Plan on arriving 15 to 20 minutes before your appointment to complete  any necessary patient registration.

No-Show

If you need to miss an appointment for any reason, please contact Mercy immediately. Appointments canceled less than 24 hours in advance will be considered a no-show. Three no-shows in a 12-month time period could lead to a possible discharge

Mercy offers translation services for patients who speak any language as well as American Sign Language.

Mercy Family Walk-in clinics provide limited medical services for adults and pediatric patients with no appointment necessary every Saturday til 1:30pm and Monday-Friday until 4pm.
 
Mercy goes a step further with Saturday for the Kids. To help working families, Saturday walk-in clinic hours will include a pediatric provider, so you can get the same great care 6 days a week.

Infections, sinusitis or bronchitis

What To Bring

  • One form of photo ID (driver’s license, passport, state-issued ID)
  • Current health insurance card, if applicable
  • Proof of income to apply for our Sliding Fee Discount Program
  • A list of current medications including the dosage. Please bring the actual prescription bottles.
  • Previous medical records, if you are a new patient
